Findings and practical limits
The report records the facilities checked, responses received and limitations. A canvass is a lead-development process and does not guarantee access to protected records or a complete treatment history. Any further record request requires appropriate authority.
Identify the geographic and time scope
Provide the relevant dates, known treatment information and the locations connected to the assignment. A narrowly defined canvass question can be more useful than a broad inquiry without a clear purpose. Before any facility contact, the client and investigator should establish the permitted scope and any authorization or documentation needed for the proposed inquiry.
Treat responses as limited information
A facility response may be incomplete, unavailable or restricted. No information obtained does not establish that treatment never occurred, and a location lead is not a medical record or diagnosis. Reporting should identify the inquiry made and the response received without implying unrestricted access to protected health information or guaranteed access to treatment details.
